    Newcastle Freeman Hospital and Surgeon Karen Booth Investigation

    Newcastle Hospitals NHS Foundation Trust reviewed more than twenty of heart surgeon Karen Booth’s cases, between April 2018 and April 2021, after colleagues raised concerns.

    The BBC reports that the investigation identified that seven people died following multiple failures.

    Hudgell Solicitors are representing those affected. The Trust has admitted Ms Booth’s errors caused the death of Ian ‘Beano’ Philip, 54, of Blyth, during a ‘routine’ operation to repair his heart valve.

    Evidence of investigation into Post Office software may open door to quashing convictions

    The first case relating to former sub-postmasters’ convictions involving Capture accounting software, used in Post Office branches during the 1990s, has reached the Court of Appeal.

    It follows evidence being submitted by a computer expert who had investigated Capture in 1998, before the trial of Kent sub-postmistress Patricia Owen. The expert was unexpectedly stood down before giving evidence in court, however, and Mrs Owen was convicted of theft.

    His original report, which warned that ‘bugs and errors’ in the system would make any evidence ‘unsafe’, now looks set to pave the way for many other cases to be referred to the Court of Appeal.

    Rapid national maternity and neonatal services investigation

    Fourteen maternity and neonatal units in the UK are now under investigation for failings in care and safety dating back more than 15 years.

    The government’s rapid review will ‘look at worst-performing services in the country but also across the entire maternity system’. The investigation aims to deliver truth and accountability for affected families and drive lasting improvements.
